Participant Waiver and Release of Liability
Event/Program: National Excellence in Taekwondo — July 25 or August 15, 2026
Organization: Team Next USA and host school (TNT Martial Arts and Fitness or Core Taekwondo)
1. Voluntary Participation and Physical Condition
I hereby certify that I am in good physical condition and have no medical limitations that would prevent my safe participation in Taekwondo training.
I acknowledge that I am voluntarily choosing to participate in high-intensity physical activity.
2. Assumption of Risk
I am fully aware that Taekwondo is a combat sport involving physical contact. I understand there are inherent risks and hazards, including but not limited to:
- Minor injuries (bruises, scrapes, sprains)
- Major injuries (broken bones, concussions)
- Catastrophic injuries, including permanent paralysis or death
I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by me as a result of my participation.
3. Release and Covenant Not to Sue
I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Team Next USA, Christian Suh, Taylor and Tavis Smith, Douglas Townsend, Wasiu Tadjou, host schools and venues, and all instructors, staff, employees, and volunteers
(collectively referred to as "Releasees"). I agree to hold the Releasees harmless from any and all liability, claims, or demands arising out of my participation
in physical activity or presence on the premises, whether caused by the negligence of Releasees or otherwise.
4. Media and Photo Release
I grant the Releasees permission to use my likeness in photograph, video, or other digital media ("photo") in any and all publications, including web-based
publications and social media, without payment or other consideration.
5. Signature and Acknowledgement
By checking the Terms and Conditions box and submitting this form, I confirm that this constitutes my electronic signature and that I agree to this waiver.
I certify that I am at least 18 years of age. If the participant is under 18, a parent or legal guardian must provide consent and accept this waiver on the participant's behalf.
